Singapore Permanent Residents

Singapore Birth Certificate/NRIC + Re-entry Permit/Passport
(For child born in Singapore)

OR

Re-entry Permit + Passport
(For child born in other countries)

1 – Please select the nationality as “Malaysian” and input child’s 12 digits Malaysia NRIC in the application
2 - Please select the nationality as “Other Nationality” and input child’s passport number in the applicationPlease ensure the following before uploading the documents:

Child’s Birth Certificate/Passport Number on the supporting document matches with the details indicated in the application page; and

Document details are visible and legible, and the photograph is clear; and

Document must be in jpeg or pdf format, and file size must be 4MB or less.
